I was in India last November and fortunately, was not sick even once. Our tour group ate the majority of its meals at our hotel and they were all top notch. When I had to eat out, I usually chose soup because I figured it was at least boiled and/or tandoori which is baked. One becomes very aware of sanitation issues while in India so it`s essential to use hand wipes which I carried with me and also small antibacterial soap containers. I too have IBS and my doctor and surgeon had some concern about the trip and were reluctant to let me go (my doctor is Indian!), The only train I travelled on was the Shatabdi Express to Agra, site of the Taj Mahal - we were on it three hours, during which time we had 3 different meals. I would visit your doctor for recommendations but Lomotil is usually a given when travelling in third world countries. I would be interested in train trips your friend takes in India because I am assuming they would be first class. I too am absolutely fascinated with India and would love to return.
