Travel in southeast Asia

My husband and I are currently in Chiang Mai, Thailand. We are leaving early February for some travel in Laos, Cambodia, and Burma. I have been reading a lot and talking to travel agents, but would value input from people who have traveled in the these countries.
My main concern is travel between these countries and within them. What is the best (time wise, budget, comfort) way to go from Chiang Mai to Luang Prabang; LP and Siem Reap; SR and Bangkok; BGK and Rangoon; Rangoon r/t to Mandalay and/or Bagan and then back to BGK. This is our itinerary, as of today, for the one month we will be traveling, and it subject to change. Currently no visas for travel within Burma are issued at the border, so we must go to Bangkok before.
Any first hand information will be greatly appreciated. I have also saved and reread Fanny Farkas`s letters from her friend who traveled in some of these countries. They are beautifully written and helpful.
